Back when we had our own dungeon in our home, we wrote this waiver, rule sheet for the Guests of the House of DeSade.  We have published it here incase you could use parts or all of it.

House Of De Sade House Rules and Waiver

RELEASE AND WAIVER  LIABILITY, ASSUMPTION  RISK AND INDEMNITY AGREEMENT FOR HOUSE  DESADE EVENTS IN CONSIDERATION  being permitted to play, monitor, observe, work for, or participate in anyway in the EVENT(S) or being permitted to enter for any purpose any PLAY AREA (defined as any area in which bondage and discipline, Sado-masochism, or D/s activities take place), EACH  THE UNDERSIGNED, for himself, his personal representatives, heirs and next  kin:

Acknowledges, agrees and represents that he or she has or will immediately upon entering any  such PLAY AREAS, and will continuously thereafter, inspect the PLAY AREAS which he or she enters. He or she further agrees and warrants that if at any time he or she is in or about PLAY AREAS and feels anything to be unsafe, he or she will immediately advise the owners  such and will leave the PLAY AREAS and/or refuse to participate further in the EVENT(S).

HEREBY RELEASE, WAIVES, DISCHARGES AND COVENANTS NOT TO SUE the owners, XXXXX XXX, or the House Of De Sade, owners or lessees of premises used to conduct the EVENT(S), premises and event inspectors, and others who give recommendations, directions or instructions or engage in risk evaluation or loss control activities regarding the premises or EVENT(S) and each of them, their directors, officers, agents and employees, all for the purposes herein referred to as "RELEASEES”, FROM ALL LIABILITY TO THE UNDERSIGNED, his personal representatives, assigns, heirs and next  kin FOR ANY AND ALL LOSS OR DAMAGE, AND ANY CLAIM OR DEMANDS THEREFORE ON ACCOUNT  INJURY TO THE PERSON OR PROPERTY OR RESULTING IN DEATH  THE UNDERSIGNED ARISING OUT OF OR RELATED TO THE EVENT(S), WHETHER CAUSED BY THE NEGLIGENCE  THE RELEASEES OR OTHERWISE.

HEREBY AGREES TO INDEMNIFY AND SAVE AND HOLD HARMLESS the Releasees and each  them FROM ANY LOSS, LIABILITY, DAMAGE OR COST they may incur arising out of or related to the EVENT(S) WHETHER CAUSED BY THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE.

HEREBY ASSUMES FULL RESPONSIBILITY FOR ANY RISK OF BODILY INJURY, DEATH OR PROPERTY DAMAGE arising out of or related to the EVENT(S) WHETHER CAUSED BY THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE.

HEREBY ACKNOWLEDGES THAT THE ACTIVITIES OF THE EVENT(S) ARE VERY DANGEROUS and involve the risk  serious injury and/or death and/or property damage. Each of THE UNDERSIGNED also expressly acknowledges that INJURIES RECEIVED MAY BE COMPOUNDED OR INCREASED BY NEGLIGENT RESCUE OPERATIONS OR PROCEDURES OF THE RELEASEES.

Acknowledge and fully understand that each participant will be engaged in activities that involve risk of serious injury, including permanent disability and death, and severe social and economic losses which might result not only from their own actions, in actions or negligence but the action, in action or negligence of others, the rules of play, or the condition of the premises or of any equipment used. Further, that there may be other risks not known to us or not reasonably foreseeable at this time.

Assume all the foregoing risk and accept personal responsibility for the damages following such injury, permanent disability or death.

HEREBY agrees that this Release and Waiver of Liability, Assumption of Risk and Indemnity Agreement extends to all acts of negligence by the Releasees, INCLUDING NEGLIGENT RESCUE OPERATIONS and is intended to be as broad and inclusive as is permitted by the laws of the Province or State in which the EVENT(S) is/are conducted and that if any portion there is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect.

The risk of injury from the activity, “toys”, implements, and weaponry involved in bondage and discipline, Sado-masochism, and dominance and submission is significant, including the potential for permanent disability and death, and while personal discipline will minimize this risk, the risk of serious injury does exist.

I understand that the activities of bondage and discipline, Sado-masochism, and dominance and submission are physically and mentally intense. I understand the rules of play and will comply with all rules and regulations. If I observe any unusual or unnecessary hazard during my participation, I will bring such to the attention of the nearest host as soon as practical.

I understand and agree that this Release  Liability Agreement covers each and every House Of De Sade activity and events in which I participate hereafter.

I HAVE READ THIS RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK AND INDEMNITY AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT, ASSURANCE, OR GUARANTEE BEING MADE TO ME AND INTEND MY SIGNATURE TO BE A COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABILITY TO THE GREATEST EXTENT ALLOWED BY LAW.
